Thinning Skin and Retinols and Sculptra

May 27, 2009
Hi Dr Irwin, I had kenalog shots for my keloids on my arms. I didn't realised that one of the side-effects was thinning skin (I can see the blood vessels) and the skin is depressed and indent across a large area on the arm. Its been more than 8 months since my last shot. Would fillers like Restylane or Juvederm reduce the overall appearance of the indent?

I'm going to assume that you've asked your doctor about this already!!   Here are several ideas but you'll need your doctor to help you with them.  One, get a prescription for tretinoin (generic for Retin-A, Renova, etc.) either .025 or .05 percent and start putting it on nightly. It has the well documented ability (it builds collagen) to slowly improve steroid atrophy (thinning of the skin).   Use it for at least 6 months and, if you get irritated, use it a little less frequently in the beginning.

Second idea would be to find someone in your area who is experienced with Sculptra.  This is the lactic acid based injectible that helps to build collagen.  It can be used to correct depressions but would need to be done correctly!  Be sure the doctor you see is experienced.  Try the Sculptra website. It would be better than Juvederm or Restylane but try them if you don't have Sculptra in your area.   I hope it improves!
